I tend to write a bit of pre-game fluff but I have a fairly good idea of what my warbands are like these days. I would recommend easing yourself into fluff gently - don't worry about it too much in your first few games - just get the hang of the rules to begin with. As you become more experienced start writing brief summaries of what happened then as the character of individual members of your warband becomes clear start making stories about their adventures.

In one-on-one games I don't record much of what happens while I'm playing but I do take a lot of photos, they help me remember what happened later when I come to writing it up. In multi-player games I have to make notes or I'd never keep track of who killed who, or when someone got to the treasure (or whatever!).

Same as Kurgan, I tend to take a lot of pictures and then write a report alongside them afterwards. This is more convenient than taking notes while playing the battle.

Regarding fluff, I always had a lot of fluff made up for my warband while converting and painting them (I am always thinking about why the models looks the way it does and why the particular fighter would join the warband etc.). It makes reports a lot more fun to read than just account of casualties and who was moving to which position in which turn.

However, you should resolve these issues to your liking. Some people like more text and fluff, other prefer a lot of pictures with little text, etc.
