I use the (old) Games Workshop tool, some dental tools and these:

The newer GW tool is fat and stupid - no use for actual sculpting at all!
The dental tools I have were a gift from zelophahad's wife (who is a dentist!) I'll take a photo of them tomorrow.

a stiff brush dipped in water is great! can be used to smoothen any surface.
try it.

I used th gw craft knife as I'm too poor to afford the fancy things works rather well though.
